Forms & Documents

When registering your child, you will need an original birth certificate, physical exam/current immunization records, proof of residency, IEP Information (if applicable), last report card and a completed registration form.  Please refer to the forms listed below:

Proof of Residency:*

Before any student attends the Hull Public Schools, the student’s parent or legal guardian must prove legal residence in the Town of Hull.  Families whose primary residence is outside the Town of Hull are not eligible to attend the Hull Public Schools.

*Required documentation for enrollment of preek-12 McKinney-Vento (homeless), foster care and military students is an exception.

CORI

Please fill out the CORI Form and bring it to the main office accompanied with your drivers license or state ID if you or any family members plan on attending any school-related events during school hours that bring you into the classroom areas. This must be done in person.
